Baseline Concussion Testing is by appointment only.

Proof of a baseline concussion test is required by the Puyallup School District prior to participation in Basketball, Football, Soccer, Volleyball and Wrestling. Baseline Concussion Tests are valid for two (2) full years, the same as athlete's physicals.

There will be opportunities to schedule a Baseline Concussion Test prior to every sports season. Baseline Concussion Testing is optional for all other schools.

To register for a Baseline Concussion Test, you will need the following:

• Athlete's Full Name
• School
• Grade
• Sport

Cost: $15.00 - Credit Card Payment Only

Upon successful registration, an email will be sent containing a check-in code - please bring this code with you to your testing appointment. Additionally, you will receive an email copy of your payment receipt.

If you need to cancel or transfer dates/locations, 24+ hours in advance, please reply to the confirmation email or email baselinetesting@osstherapy.com          

Any cancels within 24 hours of appointment or No-Shows, will not be refunded or transferred.
